NEXT MEETING: To Be Confirmed
As many of you will know we have had a patient group at the surgery since 2011. However the format of this group has changed - please click on the tab above "Have your Say" and then select the Document tab and read "Brief Summary of PRG versus PPG".
The Patient Participation Group (PPG) will be deemed to constis of all patients (11,700 approx) but a small committee will meet to discuss suggestions/proposals etc. The PPG will run independently but Vine House will have a representative at the meetings.
If you wish to become more invovled in the PPG please sign up below and you will be kept in contact via email with regards to issues and meetings.
If you don't want to be involved in the committee but would to be kept up-to-date via our "Grapevine" newletters, then please follow the link on our Home Page. So far approximately 700 patients have signed up for this service!
If you have any questions with regards to the Patient Participation Group please email firstname.lastname@example.org
If you wish to join our Patient Participation Group (PPG), please use either the online or PDF sign-up form below. The forms incorrectly state that you will be joining a Patient Reference Group (we are now a PPG). Unfortunately I am unable to change the wording on these forms!

If you prefer, you can download the sign up form as a pdf document, print it out, complete it and return it to the practice.

The link below takes you to an Action Plan that has been running since our Group was formed in 2011. It was originally compiled as a result of looking at the data from our first patient survey report of 2011/12 and discussions held with members of our Patient Reference Group.
Action Plan v8 2015 is our most up-to-date plan following discussions at our Patient Reference Group meeting held on 14th January 2015.
ACTION PLAN 2011-2015.doc
As well as continuing to complete action points from the above plan, for 2014-15 we were asked to specifically look at three prioirity areas. The results of this work can be found under "Have your Say" and clicking on "Survey Report".
